REBA as a method to assess postural stability dysfunction
Abstrakt: Východiská: Riziko poškodenia muskuloskeletálneho systému (MSs) u zdravotníckych pracovníkov podmieňuje potrebu minimalizácie intervencií spojených s vertikálnym dvíhaním a prenášaním pacienta. Zásady bezpečnosti a ochrany zdravia pri práci sú zárukou uplatnenia správnych manipulačných techník a využívanie technických pomôcok pre manipuláciu s bremenami. Klinické pracoviská by mali byť ergonomické a navrhnuté tak, aby priame intervencie v procese zdravotnej starostlivosti nespôsobovali riziko poškodenia MSs. Manipulácia s pacientom sa uvádza ako najčastejšia príčina bolesti v lumbálnej oblasti u zdravotníckych pracovníkov.
Cieľ: Uskutočniť posúdenia dysfunkcie posturálnej stability metódou REBA (Rapid Entire Body Assessment) a identifikovať riziko poškodenia muskuloskeletálneho systému.
Súbor a metodika: Výberový súbor tvorili 4 zdravotnícki pracovníci v povolaní sestra a zdravotnícky záchranár. Metódou rýchleho posúdenia celkového držania tela (REBA) pri troch intervenciách sme určili skóre rizika poškodenia MSs a definovali intenzitu potreby prijatia preventívnych intervencií.
Výsledky: Skóre REBA bolo vyššie (10-15) u zdravotníckych záchranárov. Identifikovali sme veľmi vysoké riziko poškodenia MSs a potrebu okamžitého prijatia opatrení na elimináciu existujúceho rizika. Skóre REBA bolo veľmi vysoké (10-15) aj u sestier, ale iba pri jednej intervencii – preklad pacienta z postele na transportný vozík. Nižšie skóre rizika poškodenia MSs (8-10) sme namerali pri prekladaní pacienta z transportnej postele na posteľ v izbe a pri otáčaní pacienta na posteli.
Záver: Edukovanosť zdravotníckych pracovníkov v oblasti ergonómie práce má vysokú prioritu. Schopnosť zaujať posturálnu stabilitu a správne biomechanické polohové zaťaženie jednotlivých častí tela sú nástrojom prevencie. Riziko poškodenia muskuloskeletálneho systému sa znižujú na minimu.

Abstract: Background: The risk of musculoskeletal system (MSs) damage in healthcare workers underlies the need to minimize interventions associated with vertical lifting and carrying of the patient. Occupational health and safety principles warrant the application of proper handling techniques and the use of technical load handling equipment. Clinical workplaces should be ergonomic and designed so that direct interventions in the health care process do not cause risk of harm to MSs. Patient handling has been cited as the most common cause of lumbar pain in healthcare workers.
Aim: To assess postural stability dysfunction using the REBA (Rapid Entire Body Assessment) method and to identify the risk of musculoskeletal impairment.
Set and methodology: We used the Rapid Entire Body Assessment (REBA) method for three interventions to determine the risk score of MSs damage and define the intensity of the need to receive preventive interventions.
Results: REBA scores were higher (10-15) in paramedics. We identified a very high risk of damage to MSs and the need for immediate action to eliminate the existing risk. REBA scores were also very high (10-15) in nurses, but only for one intervention - transferring the patient from the bed to the transport trolley. Lower risk scores for damage to MSs (8-10) were measured when transferring the patient from the transport bed to the bed in the room and when turning the patient on the bed.
Conclusion: Educating healthcare workers about occupational ergonomics is a high priority. The ability to assume postural stability and the correct biomechanical positional loading of different body parts are tools for prevention. The risk of damage to the musculoskeletal system is reduced to a minimum.
